This form is for students who wish to appeal a dismissal based on Satisfactory Academic Progress (SAP) for both Academic and Financial Aid purposes. If you believe your performance was affected by serious and unexpected circumstances, you may submit this appeal for review. Please complete all sections clearly and honestly, and attach all required documentation. Incomplete forms may delay or prevent review.

Academic SAP appeals must be submitted within 5 calendar days of receiving your dismissal notification.

Financial Aid SAP appeals must be submitted within 10 days of notification.To ensure full consideration for both, you are strongly encouraged to submit this form within 5 days.
